Patient Navigator (Full Time)

SouthCoast Health Is Looking For A Patient Care Navigator For Our Imaging Department

Serves as navigator for patient and family members through coordinating care with all members of the healthcare team including physicians, clinic support staff and other members of the healthcare team.

Qualifications:

Knowledge of general medical office procedures.
Knowledge of various imaging modalities and imaging studies.
Knowledge of medical terminology.
Knowledge of reading and interpreting imaging orders/reports.
Ability to communicate clearly and speak concisely.
Ability to exercise professionalism in dealing with all levels of personnel, patients, and patients' families.

Responsibilities:

Serves as a liaison between the imaging center, primary care, and surgery centers and other agencies that provide direct and support services for imaging patients.
Reviews medical records to determine if follow up care or additional imaging is needed and that all imaging was completed per orders.
Coordinates patients' appointments including office, specialists, and testing, pre-operative, post-operative and surgical appointments.
Works referral calls to ensure streamlined access to care.
Appropriately and accurately scans and indexes documents, using correct naming conventions, into patient EHR.
Reports to work on a regular and consistent basis.

Education/Experience:

High School Diploma or GED. Associates or Bachelor's degree in medical field highly desirable. Three (3) years' experience in diagnostic imaging center. Have experience working in a high volume and busy medial/imaging practice.

Requirements:

Certifications or Licensure in one of the following: Medical Assisting, LPN, RN or RT.
