SouthCoast is looking for Full-Time LPN/MA's for our Chatham Center Location
Qualifications:
- Able to assess patient's general condition, particularly the acutely ill patient and ensures that needs are met.
- Able to assists the physician(s) with examinations, diagnostic procedures, and treatments.
- Provide direct patient care as ordered by provider.
- Maintain confidentiality in matters regarding the clinic, its patients and employees.
Responsibilities:
- Accurately documents all patient observations, treatments and procedures administered, patient/family responses, medication dispensed or prescribed, hospital data in the medical record.
- Promotes wellness by providing patient education materials, communicating physicians' advice and instructions on how to self-managed care.
- Coordinates patient care by triaging patient phone calls regarding urgency of appointments, and scheduling hospital admissions. Processes all patient referrals and required follow through. Maintains and keeps updated referral report.
- Schedules lab work, specialist visits, completes necessary paperwork to ensuring proper billing, completes physician orders. Verify and update patient data. Assists patients with completion of paperwork, as required.
- Assists with patient inquiries/concerns regarding medical care, medication instructions and prescription call-ins in a prompt, courteous and efficient manner.
- Assist with patient plans of care and document care coordination and interventions while integrating quality imitative measurements and other accrediting body requirements in order to demonstrate results and show quality improvement in patients.
- Coordinates care with hospital, home health services, hospice or other institutional settings per provider's order.
- Administers immunizations and injections after reviewing immunization record, potential allergic reactions, identifies and reviews side effects and treatments. Perform venipunctures, capillary punctures, specimen collection/preparation/testing and other lab functions as requested.
Education/ Experience:
LPN: Graduate of an accredited school of practical/vocational nursing program. Or
MA: Graduate of an accredited Medical Assistant training program, medical assistant in the military (DD214), EMT-A, 2 years clinical MA experience and certified nationally, or paramedic is required. National Clinical Medical Assistant certification highly preferred.
Minimum of one year clinical experience, preferably in a hospital or office practice setting.
Requirements:
Current Georgia LPN licensure. Must pass proficiency test. Current BLS/CPR Certification
