Counselor - Care Coordinator  (Regular Part-Time, Hourly)

Hours: 24 hours per week; Clinic hours are Monday, Tuesday, Wednesday, and Friday, 9:00 am - 5:00 pm; Thursday, 12:00 pm to :00 pm.

Pay Rate: $24.50 - $25.96 per hour

Education: Bachelor’s degree in a mental health-related field such as Psychology, Social Work, or Counseling required

As a Care Coordinator, you will:

  • Work closely with various providers in the region or system to facilitate access to program services for those identified as the target population to be served.  
  • Conduct case management, linkages and referrals to needed supports in the community.
  • Act as a liaison between the clinic and community partners to facilitate referrals and coordination of care for existing clients.
  • Assist the client in meeting client service plan goals, prioritizing areas of need, and steps needed to reach goals established, as well as identified areas of strength.
  • Engage and follow up with disengaged clients.  

Additional Qualifications and Skills

  • At least one year of related experience and/or training
  • Must be able to work in a fast-paced and collaborative environment
  • Proficient computer skills, including MS Office
  • Able to pass criminal/background clearance

Douglas Young Wellness & Recovery Center is one of our Adult Outpatient programs (AOP) which provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, and peer support. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders.

All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness.
