Located on the Eastern Shore of Maryland (Ridgely); An easy 35-40 minute commute from Dover, DE, Bridgeville, DE, and Trappe, MD, and roughly 45 minutes from the Bay Bridge.

+ Education and Experience
High school diploma or GED required, Associates degree in related field, preferred.
Must have valid driver’s license and cleaning driving record.Must be able to use personal vehicle in the execution of job duties and provide required insurance documentation of personal vehicle.
Any certifications necessary for the job will be paid for and coordinated through Benedictine
+ Key Responsibilities
Establish and maintain competent, trusting relations ships with people supported and other stakeholders.
Assist with routine medical appointments.
Communicate with RN-Case Manager about the health status of people supported.
Provide transportation to and from medical appointments as assigned.
Support the person during appointments to feel comfortable and interact with community medical providers.
Ensure all required paperwork is completed and submitted to RN-Case Manager in a timely manner.
Facilitate any follow-up required, including ensuring medication refills.
Communicate with residential DSPs, families and Program Coordinators as needed following each medical appointment and as requested by RN-Case Manager.
Assist with other, non-medical, appointments as needed.
If needed, incorporate and follow behavior protocol for each individual as it fits into his/her plan.
Meet with each individual, family and their Program Coordinator if requested, prior to Individual Plan meeting to assess and make recommendations for the individual's goals and desires for health.
Attend each individual Plan (IP) meetings as requested to work with team members chosen to help him/her achieve further success.
May stay with person supported in group home when deemed sick or injured by RN-Case Manager.
+ Working Conditions
Hours of work
Full time, day shift, 8am - 4pm, Monday thru Friday, some evenings/ weekend hours are required based on needs of people supported.
Overtime expectations
Overtime is not required but is regularly available and much appreciated. Any employee can request extra hours, regardless of seniority
Work environment
Office, Group Home, and Ridgely Campus.
Travel requirements
Provide transportation to and from medical and non medical appointments, utilizing fleet vehicles.
On-call responsibilities
Not required for this position.
