FT Position Available Mon-Fri 1st shift

Successful candidates:

  • High school graduate or GED required
  • Kansas Certified Nurse Aide Certification and Kansas Home Health Aide Certification
  • Current BLS and driver's license
  • Be customer-oriented and possess the ability to work independently with minimal supervision

Essential Functions:

  • Perform basis assessment of patients and provide services that are included in the patient's care plan
  • Report measurable outcomes as requested and communicate and changes in the patient's medical, nursing, rehabilitative, and social needs to the hospice nurse
  • Provide support and education to the patient/family and/or caregiver about the patient's disease process
