This is a full-time day position Monday thru Friday 8:00-4:30pm-Must be bilingual

Job Summary:

The CHW uses their expertise about the community they are working with to provide education about  chronic disease prevention and treatment.

Essential Functions:

• Engage in community outreach and education about chronic disease screening and health
• Learn and apply clinic policies and procedures to Care teamwork
• Educate community about chronic disease prevention and management
• Engage in relationship-building discussion with patients
• Log enrollments, educational activities, and referrals
• Track and follow-up with patients as part of clinical workflows, supporting team-based care

Position Qualifications:

• Have the ability to work independently with minimal supervision
• Possess the ability to encourage others to seek opportunities for different and innovative approaches to addressing problems and opportunities
• Great written and oral communication skills and be detail oriented
• Must have current driver's license and reliable transportation

Skills & Abilities

Education-High school diploma or equivalent

• Complete Community Health Worker Certification Course within 6 months of being hired
• Complete Blood Pressure Self-Monitoring Program training and/or Diabetes Prevention Program lifestyle coach training as designated
