CASE MANAGER

Job Purpose: The Case Manager is responsible for assessing patient’s needs and working with counselors and other care providers to design treatment plans, set up referrals and choose the most suitable resources for a patient’s specific mental health needs. The case manager will also be responsible for follow up and documenting progress on each patient. This position will also develop and provide behavioral health education to individuals, groups and the surrounding communities and conducts limited community assessments.
Patient-Centered Medical Home Essential Functions:
1. Patient Population Management:
a. Prepares patients reports in a timely objective manner, documenting all activity and communication by providing a comprehensive written report as required by company guidelines.
b. Reviews reports daily (Manage Clinic Schedule to help coordinate care for the patients coming in for various services).
c. Contacts all unreached patients to confirm that the patient will be present at his/her appointment.
d. Assists patients in navigating the health care system. Coordinates specialty care, follows-up on test results and other care coordination needs.
e. Evaluates ongoing patient progress and documents in EMR.
f. Maintains and coordinates data with our Quality Department to support the various Behavioral Health Programs (BHC/Integrated Care, MAT, and Psychiatry).
g. Works closely with the entire Behavioral Health team, providers (PCP’s, LMSW’s/LSCSW’s, BHC’s), medical assistants and other departmental care coordinators to provide the best care for the patient.
h. Reviews data reports and reporting to the various department meetings on various measures
2. Care Coordination:
a. Reviews the initial evaluation/assessment of client to determine rehabilitation plan and goals. Initial evaluation to include medical history, vocational/educational history, behavioral/psychosocial/motivational status, financial status, with plan/recommendations for rehabilitation services to reach suitable goals.
b. Develops, implements and carries out comprehensive case management plans through a collaborative process of assessment, planning, facilitating and advocacy (in conjunction with the client’s physician) that are specific and tailored to a client’s individual needs.
c. May assists in developing effective therapeutic approaches to client care.
d. Provides case management services as indicated by the circumstances of the case. Such services include meeting or corresponding with physicians and treatment providers as necessary to obtain medical recommendations, making appropriate recommendations to facilitate and coordinate treatment plan as indicated.
e. Monitors ongoing client needs through the continuum of care including additional local, regional, and national resources as needed and updates/modifies case management plans appropriately.
f. Reviews care plans and adds this information to huddle sheets as needed to support the BHC.
g. Refers patients to necessary outside resources to help support his or her various needs. Help enroll Kansans in Medicaid, Medicaid CHIP, and SSA benefits, as needed.
h. Complies with all applicable federal, state, local and clinic rules, regulations, protocols and procedures.
i. Complete all required documents for Early Detection Works (EDW)
j. Obtain appropriate signed consent forms from client to protect client’s confidentiality.
k. Provide an overview to client of program goals/objectives for client care and program rules and client obligations.
l. Define specific goals and objectives for each group session.
m. See that treatment plans have specific measurable goals and objectives and are individualized for each client that meets that person’s own needs and personal goals.
n. Coordinate with treatment team the referrals to 12-step groups/sponsors, other self-help groups, placement with therapist, MD, etc., for follow-up care/evaluation when necessary.
o. Assists with group therapy and other tasks as directed by the Director of Behavioral Health.
3. Self-Management Support:
a. Provide self-management support to patients by providing self-management tools and encouraging patients to self-manage and promote healthy behavior changes.
b. Documents when self-management support tools are used or provided to patient.
c. Conduct public education outreach activities, in the form of health fairs or otherwise, to raise awareness about services at HealthCore Clinic.
d. Helps support and market various Behavioral Health Program groups such SFP (Strengthening Family parenting classes, Psycho/social therapy groups, Anger Management, Drug and alcohol use education courses, and other integral key components.
e. Completes and keeps an updated binder of all current resources and external agency information to help serve as a resource for all patients of HealthCore Clinic.
4. Communication Skills:
a. Communicates with team members to ensure members followed through with scheduled appointments and reach out to patients if not and is actively involved in patient engagement.
5. Quality Improvement:
a. Attends necessary meetings to stay abreast of other patient care coordination efforts that patients may qualify for. (Internal meetings and community-based meetings)
b. Attends and participates as a team member on at least one HCC quality team initiative team such as quality improvement, safety, and/or employee recognition teams.
c. Maintains partnerships with KU Psychiatry, SACK (Substance Abuse Center of Kansas), DCCCA, and other program partners.
d. Supports the BH Department with patient chart audits to check on required documentation and information needed for reimbursement purposes.
Additional Essential Functions include the following.
1. Documentation of all work is required in HCC’s Electronic Medical Records (EMR) system.
2. Prepares agenda and maintains minutes from department meetings.

- Bachelor’s degree in social work, psychology or counseling.
- Three (3) to five (5) years of previous experience as a case manager required; an equivalent combination of education and experience may be considered.
- Previous EMR experience preferred.
- Assist with gathering data and inputting data into databases.
- Attention to detail and ability to compose and generate documents and reports.
- Ability to communicate effectively with vulnerable populations including those made vulnerable by their financial status, personality characteristics, place of residence, health status and/or age.
- Able to complete and submit required documentation on time.
- Understand and operate within policy, rules, regulations, and accreditation standards.
- Ability to work productively with internal and external customers.
- Adept at working well with team members.
