General Definition of Work

Performs work establishing relationships with, and providing support and services to, residents looking to obtain self-sufficiency by means of employment, and other work as apparent or assigned. 

This position reports to Jobs Plus Program Manager.

PAY: $17.98 - $25.16 PER HOUR

Essential Functions

To perform this job successfully, an individual must be able to perform each essential function satisfactorily, with or without a reasonable accommodation. ·         Recruits participants for the Jobs Plus Program.

·         Monitors progress and provides support to participating individuals and their families, and serves as caseworker for participants.

·         Maintains case histories, prepares written reports of services, and performs evaluations of participants' progress.

·         Researches public and private agencies to determine the most appropriate agencies to provide services to participants.

·         Arranges, organizes and/or implements onsite workshops to meet the needs of Jobs Plus participants.

·         Develops and maintains dynamic relationships with educational, job training, and social services. 

·         Works in partnership with agencies who serve on the Supportive Services team.

·         Promotes service coordination to residents of public housing.

·         Links residents to supportive services designed to increase earned income and reduce the need for welfare assistance.

·         Prepares service plans for the head of household and any participating family member and guides and supports clients in developing achievable self-sufficiency goals through the Jobs Plus Program.

 

Minimum Qualifications

Associate's Degree in Social Work, Human Services or related field, and one (1) year Case Management or relevant Human Service work experience, or a combination of equivalent education and experience.

 

Preferred Qualifications

Bachelor's Degree in Social Work, Human Services or related field, and three (3) years Case Management or relevant Human Service work experience, or a combination of equivalent education and experience.

 

Special Qualifications

Valid state identification or driver’s license in the Commonwealth of Virginia.
