Chart Auditor (Behavioral Health Records)                                                  JFK BEHAVIORAL HEALTH CENTER 

                                                                                                                      Philadelphia, PA, USA (On-Site)

 

POSITION OVERVIEW: Responsible for reviewing records for accuracy, completeness and compliance with clinical, regulatory, and agency standards. The Chart Auditor’s duties include:

•Selecting charts and using a census tracking form for monthly chart compliance audits, and completion of audit forms. 

•Evaluation of progress notes, treatment plans, and assessments to identify inconsistencies errors or omissions. 

•Tracking & monitoring the completion of unit treatment plans, admissions/discharges, and adherence to the outreach policy. 

•Ensure documentation reflects the delivery of appropriate, timely, and medically necessary behavioral health services that align with clinical guidelines and standards.

•Assist in preparing for the agency’s behavioral health program audits 

•Support the medical records clerk; performs other duties as assigned.

•Possess excellent verbal communication skills 

 

REQUIRED QUALIFICATIONS:

•High School Diploma or equivalent 

•Minimum of 2 years of experience with medical or behavioral health records, reviewing records for compliance, and working with Electronic Health Record (EHR) software 

•Agility in using standard computer programs (MS Office, Outlook) 

•Basic grasp of the rules of writing & grammar
